Mobile Integrated Health (MIH)

MIH191

The mission of the City of Mercer Island’s Mobile Integrated Health program, MIH191, is to connect community members to appropriate resources and provide care coordination for individuals with complex needs. This will help to ensure non-emergent community needs are appropriately addressed by human services rather than emergency (911) response. MIH191 will be responsible for contacting and supporting community members, working proactively in coordination with the Eastside Fire & Rescue to provide enhanced health services to our community and help keep our emergency responders available for crisis calls. MIH191 is a collaboration between  Youth and Family Services and Eastside Fire & Rescue.

What is MIH191?

The MIH191 program is funded and operated by Medic One and local fire departments, in partnership with King County Emergency Medical Services. Specially trained firefighter EMTs along with the MIH Intervention Specialist proactively assist residents of Mercer Island by connecting them to appropriate resources to increase community health and well-being. MIH191 will work with and advocate for community members to develop personalized care plans and connect individuals with ongoing support within the community.

Resource Navigation

The MIH191 team is tightly woven into the City’s Department of Youth and Family Services and can assist in identifying and linking to resources. MIH191 can coordinate with Aging and Disability Services, King County Falls Prevention, and other local and county programs. MIH191 can also conduct a safety inspection of your living area and provide suggestions to reduce injury. MIH191 goals are to provide residents with tools and supports to ensure a healthier life, home, and community.

Referrals

The MIH191 team primarily accepts referrals from our emergency responders and 911 providers within the region. When first responders refer a resident, the MIH191 team coordinates with them to identify resources to assist residents maintaining independence and wellbeing.

How It Works

After the MIH191 team receives a referral or request, a specially trained EMT and MIH Intervention Specialist will visit you in your home to better understand your situation, needs and concerns. Our team works with you to create a plan to connect you to appropriate resources and provides ongoing support and follow-up services specific to your situation. This service is provided at no cost and is funded by the King County Medic One levy.
